WAIVER OF LIABILITY

*By registering for Soccer4Life programs, practices, tryouts, games, camps, and related activities, I acknowledge that participation in sports involves inherent risks of injury.

I voluntarily allow my child to participate in Soccer4Life activities and assume all associated risks. I hereby release and hold harmless Soccer4Life, its coaches, staff, volunteers, affiliates, sponsors, and representatives from any liability, claims, damages, injuries, or expenses arising from participation.

I authorize emergency medical treatment if necessary and accept responsibility for any related medical expenses.

I also grant Soccer4Life permission to use photographs and videos of the participant for promotional and media purposes.

By signing below, I acknowledge that I have read, understood, and agreed to this Waiver of Liability and Medical Authorization.

Parent/Guardian Electronic Signature constitutes legal acknowledgment and agreement.
